Ask two Londoners how many NHS-funded IVF cycles they’re entitled to and, depending on which side of the city they live, you can get two very different answers, sometimes for boroughs a few miles apart. The much-discussed “postcode lottery” in NHS fertility funding isn’t just a national problem that stops at the M25. It runs straight through London itself.

 

At one end sits North East London, whose integrated care board (ICB) rewrote its fertility policy in 2023 to unify five separate borough-level rules, some of which had offered as little as a single embryo transfer, into one standard across all eight boroughs. Under that policy, eligible people under 40 now get up to three full IVF cycles, and those aged 40 to 42 qualify for one, alongside expanded IUI funding and longer embryo storage. It’s one of only a small number of NHS areas in England that actually matches the three cycles NICE has recommended for eligible women under 40 since 2004.

 

Cross into North Central London, and the picture gets murkier. The ICB there also funds three cycles on paper, but with tighter conditions attached: a BMI of 19 to 30, no children from the current relationship, and two years of trying to conceive without a known cause before referral. That board was legally wound up on 31 March 2026 as part of a merger into the new West and North London ICB, leaving patients mid-pathway to navigate a policy transition on top of the eligibility rules themselves.

 

Head south of the river and the offer drops further. South East London funds two cycles, South West London just one, both with similar BMI and existing-children restrictions to North Central London’s. West of the centre, North West London also offers only one cycle, having deferred a full policy review pending updated NICE guidance rather than committing to an increase.

 

None of this is unique to London. Analysis published by the Progress Educational Trust in February 2026 found that only two of England’s 42 ICBs currently meet the NICE-recommended three cycles, while nearly 70% offer just one, often only a partial cycle rather than a full one. What makes London’s version of the postcode lottery notable isn’t that it exists, it’s how tightly compressed it is: five ICBs, each running a materially different fertility policy, inside a single city where patients regularly cross board boundaries for work, housing or family reasons without their entitlement following them.

 

For patients, the practical effect is that where you happen to be registered with a GP, more than any clinical factor, can determine whether you get one cycle or three, and under what conditions. Anyone who finds themselves on the wrong side of that line, ineligible on age, BMI or family circumstances, or simply out of funded cycles, is usually left choosing between an appeal through their ICB’s individual funding request process or paying privately. For that second group, the starting point is typically the same one people use regardless of the borough they’re in: comparing what patients can access via a private fertility clinic in london such as IVI London looks like, once the NHS route has been exhausted or was never available to begin with.

 

With North Central London’s merger still bedding in and North West London’s promised review yet to land, it’s a patchwork that looks unlikely to simplify any time soon, and one that’s easy to miss unless you happen to live on the wrong side of it.
